THE destruction of the Clipper Graham experimental rocket last summer was
caused by a worker failing to reconnect a hose carrying hydraulic fluid after a
routine check, according to a NASA investigation.
Supporters of Clipper Graham claimed the reusable rocket would be a cheap way
to put satellites into orbit and carry astronauts. The rocket was designed to
take off and land on four retractable legs, but as it was landing after a short
test flight in New Mexico only three legs extended. As a result the rocket
toppled over, burst into flames and exploded (This Week, 10 August 1996, p
11).
The rocket鈥檚 design placed an 鈥渦nusual burden鈥� on ground technicians, the
investigation found. The technicians were supposed to check the hydraulic
system, disconnect it to carry out other maintenance work and then remember to
reconnect it.
